Pinnacle is a growing team of therapists specializing in home care therapy for adults and seniors. We’re 100% mobile and travel to patients’ homes throughout the Dallas/Fort Worth area.
Our vision is to provide high-quality, compassionate care that is easy for patients to incorporate into their lives so that they can focus on recovering.
All our therapists are licensed in the state of Texas, and we practice the latest evidence-based treatment strategies and techniques to help restore patients to optimal health.
Leadership Team
Ben opened Pinnacle in 2006, understanding that “your patients do not care how much you know until they know how much you care.” He believes that one-on-one time with patients in a comfortable space (like their own home) helps to create a positive relationship for working together for health and recovery. Over the years, he has used this thought process and the latest evidence-based treatment strategies and techniques to develop a diverse toolbox that helps him treat a wide variety of conditions.
Ben is an active leader in the community. He was an influencing member of the Texas Delegation to the APTA when Vision 2020 was passed to move PT into becoming a doctorate profession. He is also active in his local church and has served as North Texas District chair for two terms.
When Ben is not working or serving the community, you will likely find him spending time with his wife. They love discovering new food spots, finding excuses to spend time with great friends, and honing barbeque skills. They also treasure the times they get to travel to visit their children and grandchildren, who are spread out across the country.
Danette Chastain
Administrator
Danette has been in the healthcare industry since 1987. She is a Texas A&M graduate in Medical Administration and Coding. She has worked as a Therapy Administrator, Home Health Director and has experience in working with Medicare, PDGM, coding, and Quality Assurance.
Her favorite moments on the Pinnacle team were when she was able to hear about the positive impacts on patients’ lives. She has always been passionate about helping others, being a community resource, and serving as a network liaison.
Danette has lived in Texas since 1980, is an active member of her Christian church, and regularly volunteers to help people in need. She and her husband live in the country, they have five children (through foster to adopt), three large dogs, and two cats. When Danette is not working, spending time with family, or serving her community, her favorite way to relax is working on woodworking projects. She is talented at repurposing old things and making them look new again.
Dominic Onofre, PT
Physical Therapist
Dominic received his Master’s in Physical Therapy from Texas Tech University Health Sciences Center and is a licensed physical therapist in the state of Texas. Dominic is passionate about utilizing his training and education to help improve patients’ quality of life.
When Dominic is not working, he is most likely spending time with his wife and three children, being active in his local church, and finding time to exercise. He also loves to take advantage of opportunities to serve and help others by going on mission trips.
