• We proactively manage patient charts.
This includes tracking missed visits and non-billed discharges.
• We confirm approval of patient care plans.
When there are new patients or changes in care, we get in touch to ensure we have the approval to start patient care, and have the approved frequency to see the patient.
• We provide timely notes and evaluations.
We take the time to review patient notes and submit updates within 24hrs of completed evaluation.
Patient Communication
As soon as we receive a referral, we reach out to the patient to confirm the following:
- Who we are, and how to contact us.
- Who their specific therapist is.
- Verify the "place of care" address.
- Confirm that their assigned therapist will reach out within 24-48 hours.
